    • 1). Review any records or notes you have taken on the employee's performance. Also review any previous performance appraisals to remind yourself of the goals and objectives that were set the last time the employee was evaluated.

    • 2). Rate the employee's performance using your notes and previous appraisals. Typical rating choices are unsatisfactory performance, marginal performance, meets expectations, exceeds expectations and exceptional performance. Rate the employee fairly based on communicated job expectations, not uncommunicated standards.

    • 3). List notes for improvement in any areas in which the employee did not meet or exceed expectations. When expectations were exceeded, list the actions that led to this rating.

    • 4). Give general comments and ratings for things not covered in other areas of the appraisal form. For example, if you would like to commend the employee for assisting another department, be sure to list it on the form.

    • 5). Look back over all of the individual ratings that you gave the employee to come up with your overall performance rating. Give positive comments as well as suggestions for improvement. Set goals and objectives for the employee to work on before the next appraisal.
