How to Request a Social Security Number

    • 1). Gather together two personal documents that will prove your identity, including your age, citizenship, or current work-authorized immigration status. You must have the original document or a certified copy. Photocopies will not be accepted.

    • 2). Print out Form SS-5 (See Resources). This form must be filled out entirely for you to receive a social security card. The form itself is only one page, but there are several pages of instruction.

    • 3). Find the closest Social Security Administration Office to you by using the online local office search box (See Resources). Just type in your ZIP code and click "submit." The office that you see on your screen is where you need to take your application.

    • 4). Give the clerk at the Social Security Office the completed Form SS-5, and your personal documents. The clerk may keep some of your original personal documents, but will mail them back to you at a later date.

    • 5). Receive your new Social Security card in the mail in a few weeks.
