64-year-old female with refractory angina and diffuse 3-vessel disease in whom percutaneous myocardial laser revascularization was attempted. During the procedure the patient developed severe angina, hypotension, and ST-T wave elevations in the anterolateral leads. Echocardiography was performed in the cath lab.

Echocardiography shows:

  1. Pericardial effusion and anterolateral dyskinesia.

  2. Partial papillary muscle rupture.

  3. Pseudoaneurysm.

  4. Intramural cavity in the anterolateral region and dyskinesia.
