    • 1). Fill out and print an ID theft affidavit if you are the victim of ID theft. The form must be witnessed by a notary, police officer or witness. To obtain a PDF copy of the ID theft affidavit, visit the Federal Trade Commission website at ftc.gov. From the homepage, click "Identity Theft" on the right-hand navigation bar's "Quick Finder" section. On the next page, click on "Consumers," and select "Tools for Victims" in the drop-down menu. On the next page, scroll down to the "ID Theft Affidavit" section and click on the "ID Theft Affidavit" link to bring you to the form.

      If you are the victim of domestic violence or an identity theft victim, file a police report with your local New York law enforcement agency and make a copy of the report.

    • 2). Write a request letter to all three major credit-reporting bureaus: Equifax, Experian and TransUnion. In your request letter, you must provide your name, Social Security number, current address, date of birth and a list of all residences for the past five years. You must also include a photocopy of an identification card to prove your identity, and a photocopy of a utility or phone bill mailed to your address to verify your current mailing address.

    • 3). Send to the addresses below your request letter, ID and address verification photocopies, and a check or money order for $5 to each credit-reporting agency via certified or overnight mail. If you are the victim of ID theft, include a copy of your ID theft affidavit or your police report, which waives the $5 fee. If you are the victim of domestic abuse, include a copy of your police report to waive the $5 fee.
