How to Report Social Security Abuse in Ohio

    • 1). Complete the fraud report on line form. This form is available on the SSA website. In order to complete the on line form, you need the suspect and victim’s information, including their name, Social Security number, date of birth, address and telephone number. Provide a detailed statement of the events surrounding the fraud, including the suspect’s actions, where it happened, when it happened, how it happened and the reason why it happened. You should also provide the names of any other individual who has knowledge of the situation. All this information is requested to complete their fraud investigation. However, if you do not have some of it, you can still file the complaint.

    • 2). Call the Fraud Hotline at 800-269-0271. This hotline is available from 10:00 a.m. to 4:00 p.m. Eastern Standard Time. If you cannot get through on the Fraud Hotline, you can also call the Social Security Administration at 800-772-1213 and file your complaint. The information is forwarded to the OIG for investigation.

    • 3). Draft a fraud complaint letter outlining the details of your complaint. In your letter, outline the situation, including the suspect and victim information, what, where, when, why and how the fraud occurred and the names of additional parties who have knowledge of the fraud.

    • 4). File your letter with your Social Security Administration office in Ohio. You can find a list of SSA offices in Ohio on the SSA website (see Resources). Once the SSA receives your complaint, they forward the information to the Office of the Inspector General.
