    • 1). Visit the Georgia Department of Taxation and Revenue's official website. Download and print Form 500 and the form instructions.

    • 2). Complete the first page of Form 500. You need various information, including the names and Social Security numbers of all dependents. Follow the form instructions for each line. Select "Individual" as the filing status for single returns, Letter A, or "Married, Filing Joint", Letter B, for joint returns. Enter your spouse's name and Social Security number on the form if filing jointly.

    • 3). Complete the first few items of the second page of the form. Stop at line 8 where you enter the federal adjusted gross income.

    • 4). Skip to the Schedule 3 section of the form, typically the last page. Complete Schedule 3 using the form instructions. Note the last figure on the Schedule 3 -- the Georgia taxable income you calculated.

    • 5). Return to the second page of the form. Skip from the first line after the federal adjusted gross income you inserted to the line asking for the Georgia taxable income, lines 9 through 14. Do not enter anything on these lines.

    • 6). Insert the Georgia taxable income you calculated on the last line of the Schedule 3 on the correct line of the second page of the form. Complete the rest of the form using the form instructions.

    • 7). Sign and date the form. Ask your spouse to sign and date the form if filing a joint return. Place your W-2 forms and other tax documents behind the return and Schedule 3 but do not staple.

    • 8). Make a check payable to the Georgia Department of Revenue for taxes you owe. Include the check with the form. Mail the form, check and documents, only if you owe taxes, to:

      2D 500 and 500EZ (Payments)
      Georgia Dept. of Revenue
      P.O. Box 105613
      Atlanta, Georgia 30348-5613

    • 9). Mail the form and documents, if you're due a refund, to:

      2D 500 and 500EZ (Refunds)
      Georgia Dept. of Revenue
      P.O. Box 105597
      Atlanta, Georgia 30348-5597
