    • 1). Schedule a visit to your doctor's office. To be eligible to donate eggs, you must be healthy and free of any diseases or health problems. Your doctor will determine if you are healthy enough to become an egg donor.

    • 2). Contact one of the donation centers below to schedule an appointment. The requirements to be an egg donor are: You must be between the ages of 21 and 32, be in good health, height/weight proportionate, have no genetic disorders or sexually transmitted diseases, be a non-smoker and have reliable transportation.

    • 3). Complete the donor application form. Upon arrival for your appointment, you will be required to fill out a donor application, a pre-screening evaluation and a form of consent for medical tests.

    • 4). Complete the medical screening, psychological evaluation and physical. If the results from your pre-screening indicate the possibility of becoming a donor, you will continue to the medical exam and testing portion of the process.

    • 5). Sign the donor agreement form. This form is a legal contract between the donor and the center. You will be matched with prospective couples for compatibility. Once you have been accepted by a couple, you will begin the physical portion of the process -- which includes injections to stimulate egg production -- and schedule appointments for collection.
