    Part I

    • Part I of the PMP application, also known as the Experience Verification Form, requires that an applicant list the project title, project start and completion dates, project role and the primary industry involved. An applicant also must list the job title, name, address and phone number of the organization and a contact person.

    Part II

    • According to PMI, in this project description section of the PMP application, an applicant must document the number of hours that have been completed in the five domains of project management responsibilities: initiating, planning, executing, controlling and closing. Tasks that describe each domain are provided.

    Part III

    • Part III of the application requires that applicants document project outcomes. The five domains found in Part II also are listed in this section.

    Tip

    • For each project that an applicant lists on the application, Parts I, II and III of the PMP Experience Verification Form must be completed. Thus, if four projects have been listed, then these three parts must be completed four times.
